Position Purpose:
The diabetes educator provides individualized diabetes self-management education/training to individuals
and groups according to the current Scope of Practice, Standards of Practice, and Standards of
Professional Performance for Diabetes Educators. Diabetes self-management education and support
(DSMES) is an interactive, collaborative, and ongoing process through which people with or at risk for
diabetes gain the knowledge and skill needed to modify behavior and successfully self-manage the
disease and its related conditions. The primary goal of diabetes education is to provide knowledge and
skill training that help individuals identify barriers and to facilitate problem-solving and coping skills to
achieve effective self-care behavior and behavior change. Additionally, the Registered Dietitian will
provide Medical Nutrition Therapy services.

GENERAL SUMMARY:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES MAY INCLUDE
Assessment
The diabetes educator conducts a thorough, individualized assessment of the person with or at risk for
diabetes. The assessment process requires ongoing collection and interpretation of relevant data.
Goal Setting/Outcome identification
The diabetes educator works with the person with or at risk for diabetes to identify mutually acceptable
goals (DSMES outcomes). The goals/outcomes reflect information obtained through the assessment
process. These goals/outcomes serve as a tool to evaluate progress toward individual, program,
institutional or community-level goals. Goals should be specific, measurable, attainable, realistic, and
timely.

Planning
The diabetes educator develops the DSMES plan to attain the mutually defined goals to achieve desired
outcomes. The plan integrates current diabetes care practices and established principles of teaching and
learning. The plan is coordinated among the diabetes healthcare team members, the person with or at risk
for diabetes, his or her family and other relevant support systems, and the referring provider.
Implementation
The diabetes educator provides DSMES according to the defined plan and desired goals and outcomes.
Implementation may involve collaboration with other professional and community resources and services.
Provides educational interventions that utilize primarily interactive, collaborative, skill-based training
methods and maximizes the use of interactive training methods.
Evaluation
The diabetes educator evaluates individual outcome measures for each person with diabetes, aggregate
outcome measures for the program, and the quality and outcomes of DSMES.
Collaboratively develops an individualized follow-up plan with each program participant
Evaluates effectiveness of educational services provided by measuring attainment of learning objectives
Conducts a follow-up assessment upon completion of DSMEP program services, using outcome
measures from the AADE7 Continuum of Outcomes framework
Utilizes a team approach to provide services and collaborates and communicates with team members
when needed.
Documentation
The diabetes educator establishes a complete and accurate record of the client's DSMES experience and
follow-up self-management plan.

Contribute to Diabetes Community Advisory Committee and participates in other committee
functions as appropriate and assigned.

Prepare and organize written reports as applicable and in a timely manner including
electronic records, monthly data reports, Community Benefit reporting, and others as
assigned.

Consults, speaks, serves as a resource person to employees, physicians, and community
members/groups on diabetes management, and related patient education.

● Participates and/or leads committees/special projects as assigned.
● Behaves in accordance with the Mission, Vision and Values of SJMHS.
Assumes responsibility for performance of job duties in the safest possible manner, to assure
personal safety and that of coworkers, and to report all preventable hazards and unsafe
practices immediately to management.

OTHER FUNCTIONS AND RESPONSIBILITIES
Performs other duties as assigned.
REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Education: Minimum Bachelor's degree, masters preferred in related field such as dietetics, nursing, social
work, pharmacy, etc.

Licensure/Registration
Holds a current license or registration as a health professional in the State of Michigan or professional
organization, i.e. Academy of Nutrition and Dietetics.
Educator holds certified diabetes educator (CDE) credential or is CDE eligible
Training and Experience
Minimum of 6 months experience providing diabetes education preferred.
REQUIRED SKILLS AND ABILITIES
Public speaking and group teaching/presentation experience preferred. Documented knowledge of and/or
experience with chronic disease management and disease self-management educational processes.
